REGISTRATION & FORMS
To register for Camp Trillium, you can:
- call the head office (1-888-999-CAMP or local 905-527-1992) and we will mail/ email you a registration package
- Print out the registration form below and accompany it with the completed necessary medical forms.
The following forms are in PDF format. PC users simply right-click on the links and select Save As or Save Target As. Mac users control-click on the link and select Download Linked File. If you do not have a PDF reader you can get one from the following link.
- Technology and Communication Letter
- 2012 Spring Registration - This is the required form in order for your family to attend Spring Camps.This also includes our Monthly Youth Groups..
- 2011/2012 Child Camper Health Record - This is the required medical form that needs to be filled out for each child who will be attending a Camp Trillium program in Summer 2011, Fall 2011, Winter 2012 or Spring 2012.
- 2011/2012 Parent Camper Health Record - This is the required medical form that needs to be filled out for each Parent who will be attending a Camp Trillium program in Summer 2011, Fall 2011, Winter 2012 or Spring 2012.

- Camper Packing List - This is the recommended packing list for both Residential and Family Camp campers.
